If you prefer to order by fax, please call Customer Service at 800-874-5278 to register your fax number. An order form will be provided for you and all orders will receive an automatic fax confirmation.

Order requirements

• No minimum order required.

For office orders only (non-Direct Ship to Patient orders):

• Orders of four (4) or more revenue multipacks will be shipped free of charge.
• Orders less than four (4) revenue multipacks will incur a shipping charge of $5.95 per order.
• Any combination of revenue product, regardless of brand, can be combined to achieve the minimum ordering requirements.

Please provide the following information for all phone and fax orders:

• Name
• Account number
• Product/brand description
• Quantity
• Lens power
• Base curve, if applicable
• Add power for multifocal lenses
• ADD cylinder and axis values for astigmatism lenses
• Payment

Delivery options & costs

• 3 business day UPS office orders: Free shipping on four (4) or more revenue multipacks or $5.95 for orders less than four (4) revenue multipacks.
• Overnight UPS office orders: $10.00 ($17.50 for Saturday delivery) up to 25 multipacks.
• 5-7 business day UPS ACUVUE^® Brand Home Delivery: Free shipping on an annual supply* or $5.95 for orders less than an annual supply.
• Overnight UPS ACUVUE^® Brand Home Delivery: $13.95 ($19.50 for Saturday delivery).

*only 12 30-packs or 4 90-packs are needed to qualify for free shipping

Terms and conditions of sale

Payments are due by the 15th of the month in which your statement is received.
